To contact us, we are in the Boxgrove Medical Art Centre at Ninth Line and Hwy. 407 on the main floor. There is ample free parking near the building. We share an office with the Markham Hearing Centre and Dr. Dupe Oyewumi – Otolaryngologist.

Patients must bring their health card and list of medications to every appointment. If a translator to English is required the patient must provide one. All cancellations with less than 2 full business days notice will require an administration fee (see fee schedule for non-OHIP services) to be paid before the appointment will be rescheduled.
